POPULATION HEALTH PLAN
Participation in the Population Health plan covers the cost of the GAP Plan; which in turn covers employee out of pocket. Deductibles and coinsurance are covered at no cost to the employee by a combination of paycheck pre tax purchase and direct monthly reimbursement of excess premium for those participating in the Population Health/Wellness plan. A high deductible plan becomes Platinum.
